YARD SERVICE CLAIM CHECKLIST

The following is an exterior or yard service claim checklist to assist you with the tracking and activities involved in the handling of your claim. This document is designed for your use in understanding the process and so you can help us expedite the processing of your claim. Click here to view the checklist for interior claims process.


Yard Service claim process checklist

  • Initial call to the CPRC Yard Service Department will prompt either the mailing of a Claims Eligibility Form or the creation of a claim from information taken over the phone.

    The steps are different for the two processes and are outlined below:

  • You will be mailed a Claims Eligibility Form if your line has already been replaced or a repair occurred more that 5 days ago. Please note the specifics of this process:

    • The Claims Eligibility Form takes approximately 7 to 21 days to reach you unless you have obtained a form through this website.

    • Once you receive or print the forms, please fill them out completely and mail them back with any receipts for replacement or repair to the line. If the plumber saved you a section of the polybutylene pipe, please send in no more than 12 inches of this pipe with your paperwork. You must also include documentation on when the home was built or the original date polybutylene was installed. (Please keep a copy of this information for your records.)

    • Once received, we will process the paperwork and assign a claim number to your claim. This usually takes 30 to 45 days.

    • If it is determined that the claim is qualified, you will receive a letter or phone call regarding the further processing of your claim.

    • If it is determined that the claim is not qualified, you will receive a denial letter.

  • If you are calling regarding a current or active leak in your yard line, the process will entail the following.

    • We will request information from you over the phone to file a claim. This will include the year the property was built or when the polybutylene was installed as well as other pertinent information. You will receive a claim number at that time.

    • Our verification and qualification process will then be applied to the claim. This involves verification of all the information you have given us regarding ownership and the year the polybutylene was installed.

    • If we cannot verify the property information from our in house data source, you may be contacted to send/fax in additional information to prove eligibility.

    • At this point the claim will either be approved for assistance or denied.

    • If the claim is denied, you will receive a phone call or letter of denial.

    • If the claim is approved, we can usually fax this information out to a contracted plumber in your area for line replacement. They will then contact you within 24 to 48 hours to set up the replacement work

  • Plumbing Process:

    • The plumbers will verify polybutylene and a leak prior to replacing the line. They will then replace the line with either Polyethylene (PE) or Poly Vinyl Chloride (PVC) at no charge to the homeowner. Copper can be installed but any additional cost for the copper upgrade will not be covered under the settlement unless copper is required by the local plumbing codes. The copper charge will have to be paid directly to the contractor at the time the work is done.

    • The plumber will have paperwork that you are required to complete before your line can be replaced. These forms include a Claims Eligibility Form and a Work Authorization Form. The contractor will also furnish you with a Scope of Work outlining how the work shall be preformed under the Cox guidelines. You must sign the Work Authorization Form as this allows the plumber the authority to do the line replacement. Failure to do so will delay this work. When the work is complete, you will have to sign a Certificate of Completion form verifying the work was done per the Scope of Work. These forms are sent back to the CPRC along with the contractor's invoice. The contractor will furnish you with a one-year warranty on the line replacement per the Scope of Work.

  • If there are any leak repair or damage expenses from the leak that need to be addressed, please notify our office. We will make arrangements to have the proper forms mailed to you or have an adjuster sent to your property. Damage issues can take up to 120 days to be resolved.
